UV Insurance needs to collect your personal information in order to provide you with the high quality services you have requested, as well as to:

- Through your financial security advisor, who collects it on our behalf;
- Directly, in the course of providing products or services we provide to you;
- Through forms on our website (www.uvinsurance.ca) and our digital interfaces, as well as:
- When you make a claim under your insurance contract;
- Your statements (administrative forms);
- When submitting your applications for insurance products (questionnaires).
What Information is Collected?
The type and sensitivity of the information we collect about you depend on the services we provide and the legal requirements we must follow.
Examples of personal information collected include:
- Name, address, email, phone number;
- Age, gender, marital status;
- Identification numbers, such as your driver’s license or social insurance number;
- Insurance coverage, transactions and service utilization history;

- Medical information, if required, for some of our insurance products.

UV Insurance requires your consent to use your information for any other purposes, to share it with third parties other than those prescribed by law or to collect additional information about you.

subject to legal or contractual restrictions.
